Covering all of Sutton Hospital Pharmacy MDU alone, remotely.

Assessed work load, sent out emails to notify leads of departments, request at least one pharmacist/pharm tech to be present in the the MDU room in hospital. Redirected all phone lines to my mobile.

Completed only required work load for on the day and next day.

Sucessful day, no delays to treatment. Handed over for when sick/annual staff returned next day and put in better contingency plan if next day would be a similar situation.

explain to requesting staff that there will be a short delay to clarify the prescription.
contact prescriber/ or medical team taking care of patient. Get medication re-prescribed, then action request.

Long wait for patient to receive oral chemotherapy from outpatients

Explained to unhappy patient and relatives cause of delays - Bloods results highlighed saftey concern . Medical team not contactable.

resolved issue by sending patient and family home. Informed patient that medication would be delivered to their home once query resolved.

next day - Dose reduction confirmed and medication sent out to patient.
