Why have you chosen to register with The Locum Agency? *

To do contract work

Tell me about a recent occasion when you were under pressure and handled it successfully. - What was the situation? - What actions did you take? - What was the outcome?

The Pharmacy Inspectors were coming to inspect the Pharmacy registers for Controlled Drugs.
I immediately checked the register and the physical stock to make sure they tally. The books that do not balance with the physical stock, I allocated to staff in the pharmacy, with deadlines to look for CD prescriptions that were dispensed but not recorded in the register. I also checked the computer for any dispensed controlled drugs that were not registered in the CD book. We then recorded all the found prescriptions that were initially not recorded in the register.
The outcome was that all dispended CD prescription were accounted for and registered in the CD book.
Decision was also made that all CD prescriptions must be recorded in the CD register on the day the prescription was dispensed.

What is your understanding of the Medicines Act 1968?

The Medicines Act 1968 is an Act of Parliament in the U.K. It regulates the manufacture and supply of medicines for human and veterinary use.
It divides medicines into POM (Prescription only medicines), P (Pharmacy medicines, and GSL (General sales list).
The Act also gives power to the courts to charge dispensing mistakes and errors as a criminal offence.
The Medicines Act 1968 was introduced to help control the use of medicinal compounds and to increase patient safety.

You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do?

Refer back to the person requesting for the medication to be prescribed and let them know that their request is not clearly documented within the patient's notes and medical record. Let them know that once the request is clearly documented within the patient's notes and medical record, I shall do the necessary on my part.

What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in?

Contact my line manager and contact my Agency to inform them of the incident or complaint. I will then explain my side of the story and look for appropriate solution to the complaint.

Please outline your understanding of Clinical Governance.

Clinical governance is a framework to ensure high-quality and safe health care. It is a systematic approach to maintaining and improving the quality of patient care within the NHS and private sector health care.

Clinical governance is involved in the pursuit of excellence. It is a framework through which NHS organisations are accountable for continually improving the quality of their services and safeguarding high standards of care by creating an environment in which excellence in clinical care will flourish.
Clinical governance is involved in recognisably high standards of care, transparent responsibility and accountability for those standards, and a constant dynamic of improvement.
Clinical governance is composed of education and training, clinical audit, clinical effectiveness, research and development, openness, risk management and information management.

Thinking about a recent difficult situation you’ve had with patient’s relatives – How did you handle this and the outcome

I explained to the patient's relative the motives of my decision making which is in relation to patient safety and well being.
