What is your understanding of the Medicines Act 1968? | The most important for pharmacist in a general perspective: - Supply of medication is restricted unless there is a license avaiable i.e., MA/PL, MS lincense, MIA(IMP), WDA etc. |
You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do? | Before any prescribing can be proceed. They need to be further clarified with relevant health professional who are responsible to looking after this patient. If this person is myself, I will need to ascertain/locate further notes and medical records for the decision to be made, including any relevant patient history on medications from other sources |
What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in? | I will make sure the incident/complaint is fully investigated, all relevent individuals have been spoken with to ensure open, honest and transparency. The immediate impact will need to be assessed and remedied right away, a corrective action agreed immediately with the goal to resolve without further escalation. Lesson learnt session is needed to prevent further similar cases from happening again The person who raise the incident or complaint will need to be inform of the process and be able to provide input on how the improvement can be made. |

Thinking about a recent difficult situation you’ve had with patient’s relatives – How did you handle this and the outcome | Note my role do not involve direct contact/conversation with pateint's relatives, however, the goal here is to respect, understand, listen, act and reassurance the issue and conerns of the relative. Also, it is important to not bleach and patient confidential issue unless the patient him/herself have given consent to do so. |
