I have relocated due to family reasons. Looking for locum roles to get a feel of jobs related to local and surrounding areas.

Situation:

- We are over capacity and my team was not able to provide all of the demand needed.
- Due to over capacity we were unable to provide a service where we are normally capable of.
- The team was very tired and wasn't able to handle any more despite all staff have contributed to staying later than their normal hours.

Actions:

- I have discuss with team to understand the issue, concerns and options
- We have as a team agreed on further actions that is helpful to situation (to setup a team over the weekend to handle the extra workload)
- We have delegate on each other agreed tasks and have agreed on how to proceed.
- As the lead of the team, I have agreed on when to re-group for collecting all the updates and I have nomainted myself to be the person to commnunicate the information to our end-users and stake holders.

Outcomes:

- End-users and stake holders were given time to react to the new and be able to voice their queries for the extra session and how does ti impact on thier service
- Further conversation with them also helped with rearranging patient appointment times to minimise disruptions to patient expereince as much as possible

The most important for pharmacist in a general perspective:

- Supply of medication is restricted unless there is a license avaiable i.e., MA/PL, MS lincense, MIA(IMP), WDA etc.
- Only exemption comes from Section 10 of Medicines Act 1968 where pharmacist are allowed to dispensed (supply) medications under certain conditions.
- This includes supplying medications under the instruction of a prescription for a specific name patient.
- The prescription itself can only be written by certain individuals (i.e., doctors or dentist) - this is updated to also included non-medical prescribers as per The medicines regulation for human use.
- Instruction from the prescription have to be followed without any deviation

Before any prescribing can be proceed. They need to be further clarified with relevant health professional who are responsible to looking after this patient.

If this person is myself, I will need to ascertain/locate further notes and medical records for the decision to be made, including any relevant patient history on medications from other sources

I will make sure the incident/complaint is fully investigated, all relevent individuals have been spoken with to ensure open, honest and transparency.

The immediate impact will need to be assessed and remedied right away, a corrective action agreed immediately with the goal to resolve without further escalation.

Lesson learnt session is needed to prevent further similar cases from happening again

The person who raise the incident or complaint will need to be inform of the process and be able to provide input on how the improvement can be made.

Clinical governance is the system through which NHS organisations are accountable for continuously improving the quality of their services and safeguarding high standards of care by creating an environment in which clinical excellence will flourish (Department of Health)

Note my role do not involve direct contact/conversation with pateint's relatives, however, the goal here is to respect, understand, listen, act and reassurance the issue and conerns of the relative.

Also, it is important to not bleach and patient confidential issue unless the patient him/herself have given consent to do so.
