Please summarise your career over the last 3 years

I have been working ina prison where I worked in the dispensary but my main job was working on the wings. I Supervised Methadone consumption also day to day medication.

Have you ever been disciplined, suspended, or are you currently under investigation by an NHS Trust, GPHC or other employer (including other agencies) – if yes, please give details

No

Why have you chosen to register with The Locum Agency? *

I like the idea of see pharmacy in different care settings.

Tell me about a recent occasion when you were under pressure and handled it successfully. - What was the situation? - What actions did you take? - What was the outcome?

I had a late in the day intake from the courts, which meant the patient hadn't seen a substance misuse doctor. Next morning when the patient came for his Methadone there was no prescription issued. I was not aware of his arrival till morning myself.
His mood changed and got very aggressive shouting at me. I explained I would personally contact the substance misuse team to issue the prescription and guaranteed he would have his Methadone. I managed to calm what was turning into a volatile situation.

What is your understanding of the Medicines Act 1968?

A license was introduced which meant there were 3 types categories Medicines.
Prescription only medication (POM) a prescription had to be issued by a licensed practitioner.
Pharmacy over the counter (OTC) medication allowed patients to buy certain medicines only when the Pharmacist was present.
General sales list (GSL) allows some medication to sold only on selected medication and complete packaging.

You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do?

I would first confirm with Pharmacist. No medication will be given until checked.

What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in?

I would first offer to make a statement. Speak with management with respect and honesty.

Please outline your understanding of Clinical Governance.

Clinical governance is a systematic approach were by organisations are accountable for continually improving the quality of care they provide and safeguarding those standards of care. This is done through things such as training, auditing and continued research, assessing risk and implementing strategies to minimise those risks.

Thinking about a recent difficult situation you’ve had with patient’s relatives – How did you handle this and the outcome

In my previous work I had no contact with patients relatives, but if I had all patient and relatives will be treated with polite respect. I will help direct relatives to the correct person they need to speak with.
