Please summarise your career over the last 3 years

For the last 3 years+ I have worked in Community Pharmacy for Well Pharmacy. I had previously been a Pharmacist Manager and now work as Relief Manager at branches across the region. My role involves all usual RP duties and leading/motivating the teams. I have provided influenza and other private vaccination services, Blood Pressure checks, Oral Contraception and Pharmacy First services in line with the role and company KPT's.
I have wanted to move my career direction to Prison Pharmacy for a considerable time.

Have you ever been disciplined, suspended, or are you currently under investigation by an NHS Trust, GPHC or other employer (including other agencies) – if yes, please give details

Job advertised for Prison Pharmacist role

Tell me about a recent occasion when you were under pressure and handled it successfully. - What was the situation? - What actions did you take? - What was the outcome?

I recently worked regular shifts at a branch in crisis. The Pharmacist Branch Manager had left the business and no replacement had been recruited. The branch has a very high workload; when the backlog of tasks became unmanageable it became a dangerous place to work in terms of risk. Staff were demoralised and patients unhappy at the poor level of service. As a consequence we were faced with daily abuse, understandably given the fact patients could not receive their prescriptions in a timely manner.

Myself and other members of the team spent the next 3 months working any extra time we could- days off and weekends. We gradually caught up with the backlog of work so that the branch could operate smoothly.

The outcome of everyone's contribution was a safer pharmacy, happier staff and most importantly satisfied customers.

What is your understanding of the Medicines Act 1968?

Governance of manufacture and supply of medicines. Licensing of all medical products that claim to have a medical function. Sets out requirements for legal sale, supply and administration of medicines.

You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do?

Contact the prescriber who initiated the medication, check for any notes in SCR, ask patient if they have current pack of said medication, contact pharmacy who may have dispensed that medication, if possible ask for copy of Rx/EPS token.

What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in?

Involve my supervisor at earliest opportunity, make thorough notes of incident, enter incident on Datix

Please outline your understanding of Clinical Governance.

The framework for healthcare organisations, whether NHS or private sector, to be accountable for maintaining and continuously improving the quality of their services and ensuring high quality of care.
It includes patient-centric care, patient safety, training, monitoring performance, and leadership.

Thinking about a recent difficult situation you’ve had with patient’s relatives – How did you handle this and the outcome

A lady came into the pharmacy and asked to speak to the pharmacist. I went to talk to her and she was very cross saying her father had been given incorrect medication in his MDS tray. She was quite loud and confrontational. I asked her if she would like to chat in the consultation room but she declined. After asking what the issue was with her father's trays I checked the PMR for all recent scripts we'd received for the patient. The scripts did not match the patient's medication record sheet but did match the backing sheets of the trays she'd returned. She stated that there were extra medications in the trays that her father had never taken. I phoned the GP surgery to check if there were any notes on their record for recently started medicines. It was confirmed that new medicines had been started following a review but the patient had been unaware of this. I reassured the patient's daughter that the medication provided was correct and showed her the last received scripts matched the trays dispensed. She left the pharmacy happy with the outcome.
