What is your understanding of the Medicines Act 1968?

Is the major pieces of legislation which governs the professional activities of Pharmacists and in particular community Pharmacist. From my understanding, this act ensures the protection of the general public in respect of medicines, by ensuring the the quality, safety and efficacy of medicines and by controlling the level of access which the general public to have them.

You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do?

Not clearly written, documented or Incomplete information in medical documentation is a pervasive issue that can compromise the quality of patient care and expose healthcare providers to legal risk. It is an essential part of GMP.
I therefore, would not process the prescription, instead will report the error to the appropriate person or section line manager, so that work can be done to investigate the root cause, and improve the quality of documentation and medical record.

What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in?

Record and gather the information the appropriate information and evidence and inform the appropriate line manager as soon as possible. I will ensure all the appropriate communications are carried out where necessary through out dealing with an investigation of the root cause.

Please outline your understanding of Clinical Governance.

It is a system approach through which the NHS organisations and private sectors are accountable for the maintaining and continuously improving the quality of their services and safeguarding high standard of care. The four main components of Clinical Governance are;
. Clear lines of responsibility and accountability for the overall quality of clinical acre
. A comprehensive programme of quality improvement activities
. clear policies for managing risks
. Procedure for all professionals groups to identify and remedy poor performance
