I have been working as pharmacy Technician specialised in all aspect of Aseptic. I have deputised in the absence of a manager led an reginal QA audit and manufactured in CIVAS, CYTO and TPN units.

I have worked closely with the senior management, pharmacist and other health care profession.

I have worked in dispensary had an exposure to both NHS and private hospital.

Tell me about a recent occasion when you were under pressure and handled it successfully. - What was the situation? - What actions did you take? - What was the outcome?

The unit have 2 isolator one morning on a busy day one of the isolator pressure was down, and since the turnover to manufacture was one hour, my first action was to start manufacturing with one isolator, then informed the manager and the pharmacist.

I have then asked the pharmacist to let me know the urgent one that I can prioritised without delaying the patient for any other treatment. once I had the list I and delegated the task to other member of staff I went back to the isolator.

Following SOP and check procedures I have found the problem. Followed the SOP and made sure that I used the right cleaning agent and written a report to make sure the team new of my action and was approved to start using the isolator the same afternoon.

What is your understanding of the Medicines Act 1968?

The act provides a licensing system when dealing in medicine even for manufacturing.

You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do?

Go back to the prescriber and make sure it is written and documented by the right people. they cant just ask to prescribe something when there is no note.

What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in?

Firstly in every work place you need to know the structure for example who is your manager and their manager all the way to the chief of pharmacy.

Any complaint which I was involve in I should be informed because this is a learning curve and I can identify weakness in in my work practises.

I would inquire about the complaint from my manager and make sure that I have learned from my mistake.

Please outline your understanding of Clinical Governance.

there are 7 pillar of Clinical Governance:

1. Patient care at the centre this is to give a good quality to the patient and that you should always remember that patient are on the receiving end.

2. Clinical effectiveness, this will help you track the treatment results and continuously evaluating clinical processes on a daily basis.

3. Patient safety, this is to make sure the safety is a top priority at all times. you can achieve this through robust reporting system, regular risk assessment and the implementation of safety protocol.

4. Governance and Leadership, this would ensure that the organisation have a string structure that can deliver the service

5. Information Management. This is to ensure that the management have implemented the correct information system to collect, analyse and disseminate data relevant to patient care.

6. Training and Education is very important aspect this continuous learning ensures that healthcare providers are equipped with the right knowledge and skill necessary to deliver high quality up-to-date care

7.performance and Monitoring, this involved the regular audits to ensure the standard is and the improvement have been implemented at all time. overall to make sure enhancement of quality of care.

Thinking about a recent difficult situation you’ve had with patient’s relatives – How did you handle this and the outcome

The relative want to know about a patient condition, which I explained that I cant do that. my first decision was to take the patient to a consultant room get my manager and we both spoke to her all we can do is to assure her that we are doing the best we can but we can't reveal more the what the patient have told us to say.
