We are looking for skilled social workers and social work leaders who have the experience of working across the Health and Social Care Services in adult social care. We are looking for candidates to join our team and be part of delivering excellent and proactive approaches to improve the outcome for adults to live as independently as possible within the community.
What’s the role?
- Discharge To Assess is an integrated multi-disciplinary service which provides rehabilitation and support to adult Lewisham residents to return them to an optimum level of function after an admission to hospital.
- The team is made up of staff from health and social care, working together to provide a holistic assessment and support plan to ensure outcomes are achieved and independence is maintained wherever possible. In this role as a Social Worker in Discharge To Assess, you will be ensuring that citizen’s with care and support needs receive community care and support in accordance with the Care Act 2014.
- Referrals are made by wards to the Discharge to Assess team (D2A team) via the Flow Centre who screen and accept the discharge passport when the relevant information has been provided, so the appropriate services can be set up to facilitate discharge.
Duties:
- To support vulnerable adults to make informed decisions and choices in their use of resources, support and services, with the aim of sustaining or improving their quality of life and well-being.
- Work as part of Lewisham’s model of Integrated Health and Social Care, with multi-disciplinary teams including the voluntary sector, to promote independence and personalisation.
- Complete an initial visit within 24 hours to ensure that the citizen is safe then decide which pathway the patient should go onto: Enablement care team (ECT) and/or supported discharge, assessment for a permanent package of care, no further input, etc.
- To undertake assessments and re-assessments in line with the Care Act 2014 – strength based assessments to implement person centred support plans and arrange, where appropriate, quality community based packages and other services for citizens and their carers
- Identify and carry out Mental Capacity Assessments and chair best interest meetings
- Protect adults at risk of abuse in accordance of Lewisham Council Safeguarding Policy, the Care Act 2014 and Making Safeguarding Personal
- To promote self-directed support and direct payments to all citizens and carers.
- Have knowledge of the CHC process and discharge to assess pathways
- Skilled report writing and manage the recording and administration around assessment and planning and agreeing care provision through up to date, thorough and accurate recording on relevant IT systems as required.
- Ensure clients are aware of processes in changing support, raising concerns about providers, complaints processes and awareness of safeguarding.
- Ensure that citizens are aware of the Adult Social Care Charging Policy and refer as appropriate for financial assessments.
- Communicate effectively and appropriately with people who have communication requirements or language barriers so they have equal access to opportunities.
