Social worker/Care Coordinator at the NELFT Mental Health and Wellness Team (south). This multi-disciplinary recovery focused team provides specialist mental health services for adults aged 18 to 70 with serious and/or enduring mental health problems. It is comprised of psychiatrists, social workers, Approved Mental Health Practitioners (AMHPs), CPNs (Community Psychiatric nurses), occupational therapists (OT’s), support time recovery workers (STR) and Peer Support workers. The MHWT also has a community psychologist who offers support on complex cases, advises on psychological issues, and oversees a clinical associate psychologist. This role also often relies on effective communication with NELFT psychiatric wards, Home Treatment Teams, and psychiatric liaison services. The role will be as a Care Coordinator or an Social Care review worker (depending on expertise/skill match). As a Care Coordinator the worker will hold a caseload of up to 25 patients and be expected their primary point of contact for support. They will also be expected to work on the Duty desk once a week where they will complete initial assessment or triage calls for those coming services and respond to patients who call the service. As a Social Care review worker, you will be completing Care Act 2014 reviews for those receiving an package of care and asked to assist colleagues with complete Care Act assessment for their patients. You may also be asked to help facilitate discharges from psychiatric hospital.
