My most recent role for the past 4 years was as the Lead Pharmacist for Clinical Services and the Deputy Medication Safety Officer at Moorfields Eye Hospital NHS Trust. My roles and responsibilities included the safe and effective provision of clinical services to patients and staff throughout the Trust which involved creating and reviewing various policies and procedures, liaising with clinicians to ensure medicines management and medicines optimisation, leading and supporting on various Cost Improvement Projects and Quality Improvement Projects, attending and in some instances chairing various meetings relating to medication safety, medicines management and audit effectiveness, including, DTMMC, Quality Forum, MSG, CAEC and MMLN meetings. I also monitored, investigated and managed medication related incidents, reviewed, actioned and managed PSAs or medication alerts received via CAS and led on the provision of appropriate training to Pharmacy, Nursing, medical and other healthcare professionals on medicines management and optimisation policies.

What was the situation: a large quantity of discharge prescriptions were received into the Pharmacy Dispensary.
What action I took: I prioritised the prescription in order of discharge times.
What was the outcome: All prescriptions were dispensed and supplied before their discharge times.

What is your understanding of the Medicines Act 1968?

The Medicines Act 1968 relates to the appropriate sale and supply of medicines depending on their licenced classifications. For example, the supply of some medicines are restricted based on the need for a prescription, whereas others are available to purchase without the need for a prescription.

You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do?

As a Pharmacist who does not have the ability to prescribe, I would not be able to prescribe any medication. Depending on what medication was requested and who the request had come from, I would potentially be in a position to refer the request to an appropriate clinician. For example, if the request had come from the patient for prescription-only pain relief medicines, I would suggest that they discuss this with the clinician that they are under the care of.

What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in?

I would speak with the hiring manager as well as my Agency Recruitment Manager.
