Tell me about a recent occasion when you were under pressure and handled it successfully. - What was the situation? - What actions did you take? - What was the outcome?

Clinical scenario

Situtation:
A hospital admitted patient had a working diagnosis of PE. Patient was prescribed LMWH as a treatment dose awaiting CTPA results.
Upon confirmation of PE, DOAC's were started and consultant didn't not agree to give a loading dose of DOAC as LMWH was given for a few days.
She insisted that a few days of LMWH will be enough to work as a loading dose and we could start the maintenance dose as she felt ok.

Action:
I double checked guidelines and manufacturer's advice and stood by it and explained to the Consultant that it depends on the choice of the DOAC and their recommended duration of a loading dose (for example 1 or 3 weeks). If we are chasing this specific DOAC and have completed 1 or 3 weeks of LMWH then it make sense to switch to the maintenance dose.

Outcome:
Consultant agreed and asked the junior doctors to listen to my explanation as it made sense.

What is your understanding of the Medicines Act 1968?

It governs manufacture and supply of medicines and the provision of dealing in medicines.

You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do?

I would double check and confirm via other multiple sources (not single) such as patient, patients relatives, pharmacy, nursing/care homes. MAR charts, previous discharges, GP records etc before confirming with the person who has written the notes/asked for it and clinically verify against indication its prescribed or intended to prescribe for and its safety to be given after checking bloods etc.

What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in?

As a professional its my Duty of candour to be open and honest with patients.
As a clinician, its my job to ensure I record any incident in incident reporting
Also bring into the attention of my line manager.

Its a framework where as an organisation (we ensure) the systems and processes are providing assurances of patient safety and quality of care and give us an opportunity to constantly improve and to safeguard high standards of care.

Thinking about a recent difficult situation you’ve had with patient’s relatives – How did you handle this and the outcome

Situation:
We were planning to discharge a patient with a life threatening condition but there was no carer at home and patient was living alone. Had a chat with a sister and she would visit her brother once a week but couldn't do it more. In view of patient safety, we had an assessment that patient had an issue with medication compliance.

Handling:
- Double checked with the chemist if he can do a compliance aid with delivery. Chemist agreed.
- Escalated to the community team to see if we can get some help with daily check on him and to see if he is taking his meds.
- Mental health referral was done in view of his mental assessment of independent living and assisting with his meds.
- Revised his medications with experts (Neurology) to see if somehow we can make it less frequent. We agreed on 50%
- Other plans including funding issues

Outcome:
Patient discharge was delayed in view of his condition and safety issues. We are still waiting to hear from the GP and district nurses if the community team would agree for them to be able to do twice a day visit to him. Fingers crossed.
