Please summarise your career over the last 3 years

After taking a big step to leave where i enjoyed working for 6 years in community/online i decided to see what was on the other side e.g hospitals or other pharmacies so i decided to some locum work which i enjoyed even more! Meeting new faces and personalities and different work load made me feel more confident. Ive dispensed, labelled worked with robots, aseptics department where i have waking IV infusion bags and end of life syringe drivers. Completed my ACT course and was checking medication before handing out and councelling them at the hospital.

Have you ever been disciplined, suspended, or are you currently under investigation by an NHS Trust, GPHC or other employer (including other agencies) – if yes, please give details

No

Why have you chosen to register with The Locum Agency? *

I enjoyed working as a locum in nottingham and as ive moved cities i think it would be a great way to back into work.

Tell me about a recent occasion when you were under pressure and handled it successfully. - What was the situation? - What actions did you take? - What was the outcome?

A patient called asking for their medication as they didnt receive it. I checked with my colleague to make sure i was checking in the right place and i tracked back the phone call through the diary we keep whenever someone calls to ask for their medication to be ser aside and for us to call once ready. It stated clearly the patient called asking for their medication two days ago. The patient in the phone was getting frustrated so i defused the situation by showing empathy and saying im going to get to the bottom this. I checked the medication slot and her medication was there it seemed to me that the patient called for medication and whoever took the call must have got aide tracked and forgot to call a call the patient. I then apologised to the patient and said they can come down ASAP to collect. The patient seemed to calm down and was happy that i showed empathy.

What is your understanding of the Medicines Act 1968?

The Medicines Act 1968 is a very important piece of legislation in the UK that regulates the sale and supply of medicines. It ensures that medicines are safe, effective, and of good quality. It also establishes the Medicines and Healthcare products Regulatory Agency to over look the regulation of medicines.

You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do?

Its important to prioritise patients safety so id Call or refer to the doctor/medical professional the double check instead of assuming and putting patients health in danger

What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in?

Report it to mangers/ higher management/ gphc

Please outline your understanding of Clinical Governance.

Clinical governance refers to the framework and processes in place to ensure the quality and safety of healthcare services. It involves various aspects such as risk management, patient safety, continuous improvement, and adherence to professional standards. Clinical governance aims to provide a systematic approach to delivering high-quality care and promoting patient well-being. It involves monitoring, evaluating, and improving the quality of healthcare services through various mechanisms, including audits, guidelines, and best practices. The ultimate goal is to enhance patient outcomes and maintain public trust in the healthcare system.

Thinking about a recent difficult situation you’ve had with patient’s relatives – How did you handle this and the outcome

A patients mum came in to collect her sons inhaler but she also had a taxi waiting for her outside so she came to the hatch and explained that the doctor has sent a prescription for an inhaler and she needs it asap for her son. I checked for the prescription and it had come through to us but after checking stock we had to go to stores to collect it which is about 5 minutes away thats where we hold all extra bulk items. I relayed this information to the lady but she started to get frustrated saying her taxi is waiting outside. I then calmly explained that she will have to wait for the inhaler as its not physically in the dispensary. She then said she cant wait and she just walked away . She didn't understand what i was trying to say that we have the medication it’s just on in the dispensary. Her attitude and lack of understanding was proving very difficult to hold a conversation.
