I have worked in Aseptics as a Releasing Officer and as a Clinical Trials Pharmacist since 2022 in The Royal Marsden, Chelsea. This role was based in oncology clinical trials and releasing chemotherapy which encompassed GCP & GMP knowledge and regulations. Prior to this role, I was working full time as a locum pharmacist working in various community pharmacies after leaving a role as a Specialist Clinical Pharmacist as The Lister Hospital (HCA - Chelsea). This role included managing the outpatient services, dispensary which involved one of the biggest IVF clinics in the UK, surgical, gynaecology and general medicine.

Tell me about a recent occasion when you were under pressure and handled it successfully. - What was the situation? - What actions did you take? - What was the outcome?

Whilst working as a Clinical Trials pharmacist, we had a brand new trial with the first patient due treatment the next morning. Upon screening and verifying the treatment, I handed in the drug to be made in advance in the aseptics unit. However the syringe connectors that were supplied from the sponsor had expired as they were sent earlier in the year. This was only picked up by the Aseptics team and relayed to myself. Given the time constraints, I liaised with the sponsor and we were able to obtain a further supply for the second treatment and also given permission to use the same standard syringe connectors that we sourced from the other site to use to make the drug. The connectors were sent via taxi and were made at a later agreed time that was arranged with the Lead aseptic pharmacist and team. This meant the drug was made in time for the patient's first treatment for the next morning.

What is your understanding of the Medicines Act 1968?

This is the law that govern the difference classes of medicines in the UK and regulates it use, provision and sale in the UK.

You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do?

I would check through any previous medical notes, check with the person requesting the prescription to confirm the medication required. I would also check if it a new medicine or a repeat of what a patient has had before. If it's a repeat, the patient's pharmacy or own medicines can be checked. If it's a new drug, the indication and reason can be checked on an electronic patient notes and other healthcare professionals e.g. the nurse looking after the patient and the doctor that reviewed the patient.

What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in?

I would speak to the people who are aware of the complaint/ incident and check the what the details and nature of the incident are, especially if there was any patient harm before deciding on any further action.

This is a set of standards of that govern maintaining and improving healthcare services. This includes aspects such as quality improvements, risk assessments and management and accountability for working practices.

Thinking about a recent difficult situation you’ve had with patient’s relatives – How did you handle this and the outcome

I have not had a recent experience of this but an example I have had in the past is when a patient has just discovered their partner had cancer. She broke down and was visibly distressed and worried. I was able to take the patient to the side and talk her through treatment her partner would be receiving and advice to help boost his immune system. I also gave her information on the Macmillian services and other support services.
