Tell me about a recent occasion when you were under pressure and handled it successfully. - What was the situation? - What actions did you take? - What was the outcome?

Working as a sole charge pharmacist, managing all pharmacist duties alone only with the support of pharmacy technicians. I prioritised tasks, ensuring more urgent tasks were completed first such as patients waiting to be seen in store, and checking prescriptions for patients waiting or returning soon. I delegated tasks to technicians and shop staff where possible, such as handing out prescriptions, processing and dispensing, answering the phone, and filing prescriptions. During busy periods, I remained calm and focused on the task at hand to ensure tasks were completed accurately and efficiently.

What is your understanding of the Medicines Act 1968?

The Medicines Act governs the control of medicines in the UK, including their manufacture and use in humans and animals. Defines categories of medicines - prescription only medicines, pharmacy medicines, and general sale medicines.

You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do?

Would not prescribe until confident the medication is meant for the patient and is going to be suitable and safe to prescribe. Could contact the patients GP/usual prescriber for verification and discuss the situation with the patient. Consider the indication of the medication and what the patient is using it for etc.

What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in?

Find out the details of the incident/complaint - who is involved/affected, when did it happened, what went wrong. Take any steps necessary or possible to rectify the situation, apologise verbally and/or in writing as appropriate to the person making the complaint. Identify what went wrong and where improvements in practice can be made to prevent future incidents.

Please outline your understanding of Clinical Governance.

The processes and systems in place to ensure that healthcare organisations and workers are continuously improving their services and delivering a high quality of care to patients. Includes audits to monitor clinical practice, risk management, continued professional development, appraisals, using compliant technology to protect patient information, clinical effectiveness and research, and recruitment of suitable staff.

Thinking about a recent difficult situation you’ve had with patient’s relatives – How did you handle this and the outcome

A woman came in very angry that we had supplied the wrong medication to her husband and accused the staff of trying to harm her husband. Although I had not been involved in the supply of medication, I received her complaint and had to deal with the situation. She was visibly upset and distressed by the situation. I calmly asked if she would like to speak somewhere private. After finding out the details of the situation I explained that her husband had been supplied the correct medication and it is usual practice to substitute a prescribed brand with a generic funded brand. I apologised that they had not been consulted about the substitution and for causing her distress in regards to her husbands wellbeing. I offered to order in the brand they were wanting and ensure that in his file it was clearly documented which brand he should receive, to ensure the error did not happen again. She accepted the apology and resolution of the situation.
