Please summarise your career over the last 3 years | I have been a pharmacy manager in private healthcare for the past 25 years, I have been responsible for the safe and effective provision of high quality pharmacy services to insured, self funding and NHS surgical in and out patients. |
Have you ever been disciplined, suspended, or are you currently under investigation by an NHS Trust, GPHC or other employer (including other agencies) – if yes, please give details | No |
Why have you chosen to register with The Locum Agency? * | I am looking to change direction slightly and expand my clinical skills to complement my managerial skills. I think that being registered with an agency may expose me to more variety of roles. |
Tell me about a recent occasion when you were under pressure and handled it successfully. - What was the situation? - What actions did you take? - What was the outcome? | We had a dispensing error within the department which resulted in a patient being discharged with medicine with the incorrect information on the label.. as a team we discussed the best way to handle this as we weren't sure which of a cohort of patients had received the incorrect pack. We decided that we would have to call each patient in turn to find out who had received the incorrectly labelled pack and once we had identified the patient we explained the correct instructions we asked them to return the medication to the pharmacy on their next visit to the hospital in order for us to correctly label it. |
What is your understanding of the Medicines Act 1968? | To provide guidance and controls on the safe management of medicinal products in the UK |
You are asked to prescribe medication which is not clearly documented within the patients notes and medical record. What should you do? | I am not able to prescribe |
What steps would you take, going forward if you become aware of an incident or complaint, which you were involved or implicated in? | I would deal with it as soon as practical, by reviewing my practice, the patients notes, or prescription, computer systems or register to ensure I understand where the incident or complaint has arisen. |
Please outline your understanding of Clinical Governance. | Clinical governance ensures guidance and a framework for ensuring patient safety, by reporting incidents, completing audit, providing training and reducing risk |
Thinking about a recent difficult situation you’ve had with patient’s relatives – How did you handle this and the outcome | Not applicable |
